We would like to welcome you as a new patient to Biologic Integrative Healthcare (BIH). We are pleased to work in partnership with you toward your optimal health and well-being. Our office staff is available to help you with questions and to ensure that your time with BIH is as pleasant and stress-free as possible.

To help us make your first visit at our facility most productive, please take the time with the following:

    Print the PDF forms below to fill out and bring with you to your
         first appointment
    Provide us with copies of any laboratory/ medical tests that you have available
    Create a contact list of your present healthcare practitioners
    Bring all prescriptions and supplements that you are taking or have used
         recently, in their original containers
    Keep a detailed diary of food intake for 1-3 days

Our facility is in the process of expanding its professional and support staff and we will collaborate with non-affiliated traditional or complementary practitioners in a patient's current healthcare team. Therefore, we can assist you in integrating those services and options which best meet your needs.
